Terms of Service
Premium Care Wellness Checks · Effective September 25, 2026
These terms cover the Premium Care Wellness Checks telephone service. The service is provided by Premiumcare, LLC, dba Premium Care In Home Care Services, a California Home Care Organization licensed by the California Department of Social Services (HCO License #334700237). In these terms, “we” and “us” mean Premiumcare, LLC. “You” means the person who signs up and pays. “Your loved one” means the person we call.
1. What the service is, and what it is not
A member of our staff calls your loved one at the times you choose, has a conversation, gives any reminders you ask for, and sends a written note to the family contacts you list. If your loved one doesn’t answer, we follow the missed-call steps described on our website and in the plan you authorize at sign-up.
The service is non-medical. Our callers do not diagnose, treat, or give medical advice. The service is not a medical alert device, not 24-hour monitoring, not an emergency response service, and not a substitute for a doctor, a nurse visit, or 911. We are only in contact with your loved one during the scheduled calls. Anything that happens between calls is outside what we can see.
2. Who can sign up
You must be at least 18 years old. You must have the right to give us the information you share about your loved one and your other contacts. Your loved one must know about the calls and agree to them, or you must be legally authorized to arrange them on their behalf.
3. Plans and prices

Any add-on you choose at sign-up, such as a second person in the same home, is charged at the price shown when you sign up. Sales tax is added where the law requires it. Payments are processed by Stripe. We never see or store your full card number.
4. Free week and automatic renewal
- Every new plan starts with a 7-day free trial. We collect your card at sign-up, but we don’t charge it during the free week.
- If you don’t cancel before the free week ends, your plan starts automatically. Your card is then charged the price of the plan you chose.
- After that, your plan renews automatically at the start of each billing period, monthly or yearly. You’ll be charged the plan price each time until you cancel.
- We’ll email you a receipt for every charge.
- If you’re on an annual plan, we’ll email you a reminder between 15 and 45 days before it renews.
- If we raise a price, we’ll email you at least 30 days before the new price applies to you. You can cancel before then.
- The free week is one per household.
5. How to cancel
You can cancel at any time, in whichever way is easiest for you:
- Online: go to Manage my plan, enter the email you signed up with, and click Cancel. There are no phone calls or forms to get through.
- By phone: (909) 284-8657, 8 AM to 6 PM Pacific, 7 days a week.
- By email: info@premiumcareca.com.
If you cancel during the free week, you won’t be charged. If you cancel after that, calls continue through the end of the period you’ve already paid for, and nothing more is charged. We don’t give refunds for partial months or partial years.
6. Changing or pausing your plan
You can switch plans, or switch between monthly and annual billing, from the Manage my plan page or by contacting us. Upgrades take effect right away and are prorated. Downgrades take effect at the start of your next billing period.
If your loved one goes into the hospital or goes away, call or email us and we’ll pause the calls until you tell us to restart them. Billing continues during a pause unless we agree otherwise in writing.
7. When your loved one doesn’t answer
We follow the missed-call steps published on our website: repeat call attempts, a text message, and then calls to your family contacts in the order you gave us. We take the final step only if you have authorized it in writing on your sign-up form. That step is either asking local police to do a welfare check, or asking a contact who holds a key to go to the home. Without that authorization, we keep calling your contacts and never contact police.
If, during a call, your loved one tells us they’re hurt or in danger, or we hear that they are, we will call 911 and then call you.
We work hard to reach someone. We can’t guarantee that we will. We also can’t control how fast police, neighbors, or anyone else responds once we’ve asked them to.
8. Your part
Please keep phone numbers, contacts, the home address, and the escalation instructions up to date. Also tell us about hospital stays, travel, or changes that affect the calls. We rely on what you give us.
9. Text messages
If you choose text updates, we’ll send messages about your loved one’s calls: up to 3 a day, plus urgent alerts. Message and data rates may apply. Reply STOP to opt out, or HELP for help. Agreeing to texts is not a condition of buying the service.
10. Limits of our responsibility
We will provide the service with reasonable care, as described in these terms. To the fullest extent the law allows, we’re not responsible for harm caused by events we couldn’t reasonably detect or prevent through scheduled phone calls. That includes anything that happens between calls, and delays by third parties we contact on your instructions. Nothing in these terms limits any right you have that the law doesn’t allow us to limit.

12. Changes to these terms
We may update these terms from time to time. If we make a change that matters to you, we’ll email you before it takes effect.
