Resident Privacy Notice
For residents, their representatives and their loved ones. Effective October 16, 2026.
In short: we use your information only to make your daily check-in call and send the notes you agreed to. We never sell it, and we don't use it for advertising. You can stop the calls, or ask us to delete your information, at any time.
What we collect
- From your community or family: your name, the name you go by, your room or address, the phone number we call, your preferred call time and language, and short notes that help your caller (for example, "a little hard of hearing").
- From each call: a short note on how you're doing that day (sleep, meals, mood, whether you took your medication if you asked us to remind you), and whether we reached you.
- People connected to you: the loved ones who get your daily note, the community staff we alert, and, if someone decides for you, your representative's name and contact details.
- Your choices: when and how you agreed to the calls and notes, and any change you ask for.
Who sees it
- Your Care Caller and their RN supervisor. Callers see only what they need to make the call. Details like your birth date, doctor and full address are kept to our owner, except when we need them to get you help.
- The loved ones you agreed to. Daily notes go only to them, and only after you (or your representative) say yes.
- Your community's staff. If we can't reach you, they get a short alert so someone can check on you. They also manage your enrollment.
- Companies that help us run the service, only for that purpose: Stripe (billing), our email provider, Twilio (texts and calls placed from our business line) and our website host.
- Emergency responders, if you have an emergency during a call or can't be reached and we're worried about your safety.
What we don't do
- We don't sell or rent your information.
- We don't use it for advertising, and we don't share it with other businesses for their marketing.
- We don't record calls.
- We are a non-medical telephone check-in service, not a doctor, pharmacy or health plan.
How long we keep it
We keep your information while your calls continue. After calls end, you or your representative can ask us to delete your call notes and personal details. We keep only what the law or our billing records require.
Your choices
- Stop the calls at any time: tell your caller, call (951) 405-3134, ask your community, or use our stop calls page.
- Stop texts by replying STOP to any text.
- See, correct or delete your information by emailing info@premiumcarewellnesscheck.com or calling us. We'll confirm who you are before making changes.
- Change who gets your notes, or stop them, at any time.
Keeping it safe
Our systems use encrypted connections, staff sign in with their own PIN, and links we send for consent or roster changes are private to the person they're sent to.
